History:

56 year old male with lumbosacral pain.

What is the most significant abnormality?

Dx: lytic metastasis to the sacrum from occult lung cancer

  

Findings: lack of arcuate lines of the sacrum on the patient's right (arrows pointing to normal arcuate lines on the left).  CT of the pelvis confirms the lesion (arrows).
